LaFargeville Central School Parents
Dear Parents:
LaFargeville School Based Health Center will be opening in January 2010. To utilize the services of the School Based Health Center, parents must complete the enrollment packet attached(SEE LINK UNDER PARENT BUTTON ON LCS WEB PAGE) and send it back to the main school office as soon as possible. We have started enrolling at this time to expedite the process when we open.
The clinic will be open from 7:30 am to 3:30 pm, Monday – Friday, when school is in session. We are staffed with a Physicians Assistant and nurse from Carthage Area Hospital every weekday open. We will offer Behavioral Health Services and Nutritional counseling on site through the clinic. All Primary care health concerns, immunizations, physicals and allergy injections are available at the clinic.
There is no out of pocket expense for any service performed at the LaFargeville School Based Health Center. Payment from insurance is taken as payment in full for services. Any service performed at the backup facility or outside of the SBHC is subject to copayment.
Please fill out your enrollment packages and return them right away to the LaFargeville School Main Office.
